To ensure a smooth booking process, we kindly request that payment be made at the time of booking through a credit card. Please note that a requisition from your primary care physician is a necessary requirement. If you do not have one, don’t worry! We can easily arrange for a referral from one of our esteemed physician partners. However, if we are unable to obtain a requisition from our partners, we will promptly refund you in full and assist you in rescheduling once the requisition is available.
